Actually "raper in anger" doesn't make any sense at all. You would say: "angry raper". "Hit and run rape her in anger" makes much more sense as a compound adjective like "one-and-only"--"hit-and-run-rape-her-in anger" (individual).
Anyway, in the end we don't know. He probably uses both according to how he's feeling. It's an ominous song about a denizen of the night/serial killer and one intent is to frighten the listeners in much the way a horror story might frighten. It is made more intense because it was based on a recent topical story at the time of writing.. Either way, it expresses the dark side of the Stones. Much like Sympathy for the Devil and Brown Sugar.
