One reason why, from what I've been told about Keith by several different people who know him, or at least used to know him, is that when it comes to his hair - he totally hates anybody cutting it, or touching it, and he has been this way since the 1960s. He flat out doesn't allow anybody to mess with his hair, even style it. He started to cut his own hair in the '60s and also would even cut some of his bandmates hair back then for awhile (there's even a photo of him doing this around '66 or so). At least as late as the mid '90s this was still the case with him and I would bet that he still is cutting his own hair today. So if he doesn't want others touching his hair, that would explain why no plugs, etc. But I don't think Keith ever cared about stuff like that either, and this is why he hasn't even colored it in years.
