It simply has to do with his body make-up. Not everybody who smokes dies from smoking cigs. It's a fact that somewhere around 15% of the population can smoke tobacco their entire lives and never be affected by it. Keith just may be in that small percentage group. I say "may be" since he hasn't died yet and time could still catch up with him and his smoking habit.
I didn't know, or at least I don't recall reading that Keith stopped drinking. Good for him if he did!
